Who is Qualified to Fill out an Assessment?
A member of one of the following professions:
- Special education teacher who is a member of the health care network;
- Occupational therapist;
- Nurse;
- Physician;
- Optometrist;
- Physiotherapist;
- Psychologist.
Choose the professional who can best describe your disability as it relates to covering distances that do not require use of a means of transportation.
You are required to pay the fees for this assessment, which are not reimbursed by the SAAQ.
Where necessary, the SAAQ may request an additional assessment.
