Registration

In order for the excellence of your behaviour to be recognized, you must first complete the registration form, where you can apply for either the Gold, Silver or Bronze excellence levels if you meet the following eligibility requirements.

• You must have travelled a minimum of 10,000 km for each of the years required for the applicable level using a heavy vehicle registered in Québec.
• You must not have committed any offences while driving a heavy vehicle.
• You must not have been responsible for an accident while driving a heavy vehicle.
• You must not have accumulated more than three demerit points in your record (for any type of vehicle), for a period of 24 months.
• You must not have been the subject of criminal charges related to driving or your employment duties.

Once the form has been completed, print and send it to the following address:

Accepted candidates will receive a confirmation by mail and their name will appear on the list along with the other drivers recognized for the level applied for.
