A simplified Claim form will be sent to you if the only accident-related expenses to be claimed are the cost of medication, damaged clothing, travel expenses, etc.
You must phone the SAAQ to have a claim file opened and be given a claim number. To obtain the guide and forms; you can: Download the documents Receive them through the mail by contacting us Go to one of our service centres NOTE: The forms are NOT available at the office of SAAQ licensing agents.	Opening a claim file: you are given a claim number, which remains valid throughout the compensation process.
