You do not have to fill out the form:

• If you are challenging a claim decision by the SAAQ before the Administrative Tribunal of Québec. You will automatically be sent a copy of documents, free of charge, in connection with the decision contested.
• If you want to open a compensation claim. In that case, click on to reach us.
• If you want general information, we can be reached by telephone or by e-mail.
• If you want information regarding an individual's claim, you must click on to reach us.
